How to Determine Mental Health Coverage
Have the following information available when reaching out to your provider: Insurance Card, Name, Date of Birth, Address, and Phone Number
Ask your provider the following questions:
“Does my plan include out-of-network coverage for mental health?”
“Is there an annual deductible and how much is it?”
“How much of my deductible have I met?”
“Is there a limit on the number of sessions and out of pocket expenses per year?”
“What is the reimbursement rate and coinsurance percentage for outpatient mental health services?”
